Gameplay is structured around a linear series of treatment steps. The player begins by selecting a patient from a waiting room. Each patient presents a specific dental issue displayed on a screen. The player must then perform a set of actions in a fixed order: first, use a dental mirror and explorer to inspect the teeth; second, apply a numbing gel; third, drill out decay using a dental drill; fourth, fill the cavity with a filling material; and fifth, polish the tooth. Operation controls are touch-based. The player taps and drags tools from a toolbar at the bottom of the screen to the patient’s mouth area. Specific gestures include tapping to activate the drill, swiping to remove plaque, and tapping to apply fillings. A progress bar indicates completion. Tactical techniques include prioritizing the removal of all visible decay before filling, as incomplete removal resets the step. Using the correct tool for each stage prevents patient discomfort, which is indicated by a pain meter. Maintaining a steady hand during drilling minimizes errors. Completing procedures quickly and without mistakes yields higher scores and bonus currency.
